Nursing Home Violations For The Month Of October

The Illinois Department of Public Health today announced the following type “A” violations of the Nursing Home Care Act were cited during the month of October. An “A” violation, which is the most serious licensure violation imposed by the state, pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result.

The statement of violation for each facility can be obtained by clicking on the facility's name.

Adloff Place, a 16-bed intermediate care facility for the developmentally disabled located at 50 Adloff Lane, Springfield, has been fined $25,000 for failure to implement its abuse and neglect policy for 15 residents. Numerous residents suffered injuries, such as cuts and bruises, and those incidents were not properly processed by staff. Residents also suffered poor hygiene due to neglect. The facility requested a hearing on the Department’s action. After a prehearing, the facility withdrew its hearing request and paid a reduced fine. Read more at idph.state.il.us

Definition:
Care management is the process through which needs are identified and managed to allow the consumer to receive service in the most appropriate, least restrictive setting. This process focuses on consumer need and includes: development of a care plan addressing consumer need; explaining options for service delivery; ongoing monitoring of service delivery; periodically reassessing the consumer condition; making the necessary adjustments to the care plan; and notification to providers of changes in service provision.
